For individuals

Individuals seeking certification as a Consultant must meet the industrial hygiene requirements of Title 24, Article 30, Section 1402​.

Online payment instructions for the individual form

Please have the following ready to upload.

  • Consultant:
    • Compliance with the industrial hygienist qualification as set forth in 24-30-1402, CRS.
    • 40-hr HAZWOPER certificate as set forth in 29 CFR 1910.120.
    • 8-hr HAZWOPER refresher (if applicable).
    • Documentation of successful completion of a department-approved consultant training course within the 60-day period prior to submitting an application.
  • Decontamination supervisor:
    • Documentation of successful completion of a department-approved decontamination worker and supervisor training course within the 60-day period prior to submitting an application.
    • 40-hr HAZWOPER certificate as set forth in 29 CFR 1910.120.
    • 8-hr HAZWOPER refresher (if applicable).
  • Decontamination Worker or Ventilation Contractor:
    • Documentation of successful completion of a department-approved decontamination worker training course within the 60-day period prior to submitting an application.
    • 40-hr HAZWOPER certificate as set forth in 29 CFR 1910.120.
    • 8-hr HAZWOPER refresher (if applicable).

For companies

An "agent for legal service" is the person authorized to receive legal notice on behalf of the company, similar to a registered agent.

Online payment instructions for the firm form

Please have the following ready to upload.

  • Consultant:
    • A letter signed and dated from agent that states:    
      • The firm will only use individuals who are compliant with the HAZWOPER training requirements specified in 29 CFR 1910.120.
      • The firm will only utilize appropriately-certified individuals to conduct assessment and sampling of methamphetamine-affected properties and the firm and its employees will follow the assessment and sampling requirements of 6 CCR 1014-3, Part 1.
  • Decontamination firm and/or ventilation firm:
    • A letter signed and dated from agent that states:
      •  The firm will only use individuals who are compliant with the HAZWOPER training requirements specified in 29 CFR 1910.120.
      • The firm will only utilize appropriately-certified individuals to conduct decontamination of methamphetamine-affected properties and the firm and its employees will follow the decontamination requirements of 6 CCR 1014-3, Part 1.

For instructors

Please have the following ready to upload.

  • All:
    • Documentation of high school diploma or GED.
  • Consultant:
    • Documentation of compliance with the industrial hygienist qualifications set forth in 24-30-1402, C.R.S., including documentation of degrees and experience claimed or instruction received. Documentation of experience claimed must include submission of employment references, including telephone numbers.
    • Documentation demonstrating a minimum of five (5) years of consulting experience. Documentation must include submission of employment references, including telephone numbers.
  • Decontamination worker, decontamination supervisor, ventilation contractor:
    • Documentation demonstrating a minimum of five (5) years of field experience in the subject(s) to be taught. Documentation must include submission of employment references, including telephone numbers.

For training providers

Please have the following ready to upload.

  • Consultant (Basic and refresher), Decontamination worker (Basic and refresher), Decontamination supervisor (basic and refresher), and Ventilation contractor.
    • A course syllabus addressing the curricula for each course as set forth in Appendix A or B of 6 CCR 1014-3.
    • Course materials appropriate to the syllabus.
    • The name and contact information of each qualified instructor who will teach each course.
    • Documentation of the qualifications of each qualified instructor who will teach each course.
