Methlab electronic payment guidance

There are three steps to pay online

  1. Enter your information into the pay portal.
  2. Make your payment.
  3. Email your payment receipt to cdphe_methlabdocuments@state.co.us.

Pay portal

For the invoice number field:

  • Enter HM08

For the customer number/reference ID field:

  • Enter the record number found in the subject line of your confirmation email. (Example: 21456591)

Make your payment and don’t forget to email your payment receipt to cdphe_methlabdocuments@state.co.us


Paying by electronic check

A flat $1.00 surcharge* will be added when paying by electronic check (e-check), regardless of the transaction amount. You will need to carefully enter your 9 digit bank routing number and your account number to complete this transaction.

Paying by credit card

A surcharge* based on the amount charged will be applied to credit card payments. We can accept MasterCard, Visa, Discover and American Express.
